Phoenix Children’s Hospital is currently conducting a research project related to ACE screening in the General Pediatrics setting and would appreciate your input. We have organized a short survey to help us characterize the current ACE screening practices in pediatric practices around the country. The survey takes 5-10 minutes to fill out. If you would also be willing to share the ACE Screening Tool in use at your practice, you can either share a description of the tool in the box in the survey, or you can e-mail it to me at mstanto@phoenixchildrens.com. If you know another pediatric practitioner who performs ACE screening in their practice, please feel free to forward this email along to them, or to send us their contact information so we can reach out directly to them. We are trying to capture the current practices around the country and are open to as many responses and insights as we can find.
Thank you for taking the time to participate in our research project. We will be accepting responses until December 15th, 2014. At that time, we will generate a comprehensive report and we will share it with all of the respondents.
