Sonoma Connect is a county-wide, community collaborative to prevent, address and heal ACEs. Providence Santa Rose Hospital was the lead applicant to ACEs Aware for a large partnership of entities committed to the vision of reducing ACEs in a generation. The collaborative received $2.97m to fund systems work that helps clinics adopt screening practices, builds capacity in clinics and within community based organization to respond to those screens, and supports the creation of permanent systems to connect people to the resources they need to be resilience.
The grant award does not fund direct services or replace other funding. Sonoma Connect is in the process of developing a website to share information continually and comprehensively, and to reach as many people as possible.
