The Multicultural Health Foundation, Southeastern San Diego Community Advisory Committee, Be There San Diego and Community Health Improvement Partners invite you to Join us at the free Data to Action event on Thursday, April 20, 2017 from 5:00 p.m. to 7:00 p.m. at the Jacobs Center. This event will celebrate our success in improving healthcare and learn about how we can work shoulder-to-shoulder to develop a Portfolio of Multicultural Health Solutions for Southeastern San Diego.
The Portfolio will shed light on ethnically diverse programs and services that are tackling the root causes of health disparities. This information will create opportunities to turn our new data into action to expand alignment of services and fill in gaps in healthcare initiatives.
Your participation in this project is crucial in producing a comprehensive Portfolio that includes:
ยท All multicultural programs and services for cardiovascular and other chronic diseases for adults and youth
- All sectors operating cardiovascular and other chronic diseases initiatives (clinical, community-clinical linkages, education, nutrition, social services, mental health, technology, policy, research, systems, faith-based, environmental, social media initiatives, etc.)
- All organizations involved in health services (nonprofits, business, faith-based, educators, government)
- Who is being served, how often and improvements or challenges in their health?
- What healthcare services are missing?
This is an invitation to ALL organizations, adults, youth and ethnicities leading actions for a healthier Southeastern San Diego. We welcome the established and the emerging grassroots organizations and individuals, those that are easily recognized and those working unseen at the community level.
