Are you finding barriers in primary care with implementation of ACEs screening and workflows? Are you experiencing barriers to information sharing when primary care and behavioral/mental health work as a team to care for a patient?
We hope to develop positive ways to eliminate these barriers at Session # 3
SCC ACES AWARE Collaborative :Peer to Peer Engagement Sessions 3-5
The 6/25 Session will focus on identifying where providers are with implementing ACE’s or what barriers exist for providers with implementing ACE’s. We will also have County Counsel come to present on HIPPA and information sharing practices.
06/25/21, 07/30/21 and 08/27/21
Join us for the continuation of our Peer to Peer Sessions.
This is a continued opportunity to meet and greet fellow Santa Clara County clinicians who are interested in or are conducting ACEs screenings in clinical practice.
During our Peer to Peer Engagement Sessions, we will discuss trauma informed efforts across Santa Clara County for ACEs screening. We hope to embed integrated and supported communities of practice, as well as to collectively (with 360 degree communication) enhance services to respond effectively to toxic stress and the risk of toxic stress to our communities.
