The purpose of this NOFO is to reduce rates of death and disability from commercial tobacco use, diabetes, heart disease and stroke, and reduce the prevalence of obesity and other chronic disease risk factors and conditions (e.g., oral health, dementia, COPD). This NOFO has three (3) separate, competitive components with eligibility and scope of work requirements for each. Applicants must submit a separate application for each component for which they are applying.
The NOFO is based on the best available evidence for policy, systems, and environmental changes, and clinical-community linkage strategies, which may be culturally adapted to meet the needs of American Indian and Alaska Native communities.
The long-term outcomes of this NOFO are:
Increased purchasing of healthy foods.
Increased physical activity with an emphasis on walking.
Increased breastfeeding.
Reduced prevalence of commercial tobacco use.
Reduced incidence of type 2 diabetes.
Reduced prevalence of high blood pressure.
Reduced prevalence of high blood cholesterol.
