Grant Proposal Summary:
This program will target patients receiving care through the county free clinic who meet the diagnostic criteria for Syndrome X and who are identified during initial intake or subsequent visits as having experienced one or more adverse childhood events defined by UNICEF (2019, p. 5) and Minnesota Department of Health (2019, para 2) as “a traumatic experience in a person’s life occurring before the age of 18 that the person remembers as an adult.” Each patient identified as meeting the criteria for participation will be assigned an initial one-hour appointment with a specialist nurse practitioner (NP). During this appointment a care plan will be developed by the patient, the NP, and anyone else who the patient wishes to be present. Plans will be individualized, but might include participating in one or more physical fitness related classes offered by the county community education department (such as aerobics and strength training, aquatic exercise Tai Chi, and yoga) each quarter, daily passes to the county-owned swim and racquet club, enrollment in local smoking cessation classes, participating in mental health support groups, or receiving one-on-one counseling. Transportation utilizing the county’s public pre-scheduled, demand-response transportation system and UBER Health will be organized in advance by the NP.
