The road to recovery doesn't end with a hospital discharge. Just ask Sejal Shah, senior care specialist at Los Robles Hospital.
I have a heart for our elderly. We all will be there at some point, the 48-year-old said. (Being) young and strong, we forget. It's my job to make sure whether the patient is here or beyond our walls, the seniors are receiving the care they need at the right place and the right time.
Data shows that by improving outpatient care, hospitals can avoid readmissions, Shah said. Nearly 20 percent of discharged seniors are readmitted within 30 days, she said.
Senior healthcare is a rapidly growing industry in Ventura County. Shah said the number of seniors over age 85 is expected to triple within the next 15 years.
Shah said watching her father battle renal failure taught her compassion and respect for seniors. Now, making sure seniors receive the care they need gives her life a joyful purpose.
