Eleven of Community Memorial Health System’s Centers for Family Health have earned Patient-Centered Medical Home Recognition status from the National Committee for Quality Assurance, a private nonprofit.

Its Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs.

The 11 Centers for Family Health are in Ventura, Fillmore, Santa Paula, Oxnard, Port Hueneme, Oak View and Camarillo. The centers provide a wide range of healthcare services, including urgent care and women’s health.
