The mission of the Yolo County Children’s Alliance (YCCA) is to assess, coordinate and act to strengthen and support the continuum of prevention and intervention services and resources for children, youth and their families. In partnership with Stanford Youth Solutions, YCCA now offers one-on-one mental health services and supports for children and youth. The Family Skills Facilitator works with youth, their families, and clinicians to develop an individualized plan focused on family strengths and the emotional, behavioral health and well-being of the youth.
Job Summary:
The Family Skills Facilitator (FSF) provides one-on-one guidance, support, and education to Medi-Cal eligible youth and their families in the youth’s home, school, and/or community based setting. The FSF is responsible for the development of Child and Family Teams (CFTs), maximizing involvement of natural team members and system partners. The FSF guides the CFT through the plan development process using High Fidelity Wraparound and Pathways to Mental Health Core Practice principles, philosophy, and service delivery techniques. In addition, the FSF may facilitate skill building groups or provide individual one-on-one rehabilitation or skill building activities with the youth as determined by the CFT and indicated in the youth’s individualized treatment plan. The FSF provides case management services and linkage to sustainable community resources and supports, as determined by need.
