This event is brought to you by Dignity Health, Sutter Health, Yolo County HHSA Community Health Branch, CommuniCare Health Centers, and Winters Healthcare who have come together to produce a joint Community Health Needs Assessment for 2019-2023 and are inviting you to the public release of the findings. We encourage all community leaders to join us to learn about the current significant health needs of Yolo County and make a commitment to working together to develop interventions to improve health outcomes!
