Hello,
I'm wondering if any mental health agencies have created and would be willing to share intake, screening, application or other forms that are trauma-informed/more user-friendly/less clinical 'jargony'.
Managed, state and federal healthcare forms tend to have certain requirements that dictate what information we need to gather, which is understandable, but I'm wondering if anyone has found some creative ways to gather this information while sounding more human to human and being more helpful to the service population.
Bonus points: Links to user-friendly community health agency websites or social media.
