Does anyone have a short questionnaire that could be administered to an organization to determine the level to which the organization/agency/school is trauma informed and trauma sensitive? Thank you.

We have used this in the past. Might be a good jumping off point for you. It's based on the Trauma Informed Care Values from Fallot and Harris:

I have one that I used for my Thesis. It is for teachers, and or administration? It has sub-sections/topics such as teacher background, teacher knowledge of trauma, brain development, effects of trauma, trauma-informed practices and self care knowledge? I am willing to forward it to you?
